Pre-admission Information

Pre-Admission and Pre-Registration 

Maternity Patients

Make your admission process faster by pre-registering. We recommend that all maternity patients pre-register (pre-admit) by their fifth month. Come by Outpatient Registration and complete the form or pick one up at your Obstetrician's office. Once you complete the process, we will send you a confirmation letter by mail. If you have questions, please call 442.7631.

Outpatients

When you schedule an outpatient procedure or test, call Pre-Registration at 442.7631 to complete your pre-admission, or come by Outpatient Registration 24 hours before your scheduled appointment.

Outpatient Registration Hours are from 6:00am to 6:00pm, Monday through Friday.

Call Pre-Registration at 442.7631 from 8:30am to 8:00pm, Monday through Friday. 

Pre-register online 24 hours a day, seven days a week...at least 24 hours prior to your appointment (EXCLUDING WEEKENDS).
